Terms of Service

1. Agreement to Terms

These Terms of Service (“Terms”) govern your access to and use of the UltraStat Imaging website at ultrastatmobile.com (the “Site”), our online booking system, and all related services (collectively, the “Services”). By accessing the Site, scheduling an appointment, or making a payment, you agree to be bound by these Terms. If you do not agree, do not use the Services.

These Terms work together with our Privacy Policy & HIPAA Notice of Privacy Practices, which is incorporated by reference.

2. Not Medical Advice; Screening Is Not Diagnosis

UltraStat Imaging provides diagnostic and preventative screening ultrasound services performed by a registered diagnostic medical sonographer. Our screening services do not constitute a medical diagnosis, do not establish a physician-patient relationship by themselves, and are not a substitute for evaluation by your physician.

Images and reports generated through our Services are reviewed by a board-certified radiologist and forwarded to you and/or your designated physician. Any clinical decision based on those results should be made in consultation with your treating provider. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room — do not use this Site.

3. Eligibility

You must be at least 18 years old to book an appointment online. Appointments for minors must be scheduled by a parent or legal guardian, who must be present at the time of service and provide consent.

4. Booking, Deposits & Payment

  • Deposit. A non-refundable $50 deposit is required at the time of online booking to reserve your appointment slot. The deposit is applied toward the total cost of your service and is processed securely through Square.
  • Balance due at service. The remaining balance is collected at the time of service via card, tap-to-pay (Apple Pay / Google Pay), FSA / HSA card, or other accepted payment methods.
  • Self-pay only. UltraStat Imaging does not currently bill insurance directly. Patients are responsible for payment in full at the time of service. Itemized receipts containing CPT codes are provided so you may submit to your insurer for possible reimbursement.
  • Pricing. Prices displayed at the time of booking are the prices that apply, except in the event of an obvious clerical error. We reserve the right to update pricing for future bookings at any time.

5. Cancellation, Reschedule & No-Show Policy

  • 24-hour rule. You may cancel or reschedule your appointment at no additional charge up to 24 hours before your scheduled start time using our patient portal or by calling (240) 909-1395. The $50 deposit will be applied to your rescheduled appointment.
  • Late cancellation. Cancellations made within 24 hours of the appointment are subject to a $35 late cancellation fee in addition to forfeiture of the deposit.
  • No-shows. Failure to appear for a scheduled appointment without notice will result in forfeiture of the $50 deposit and may result in a $35 no-show fee. Repeat no-shows may forfeit the ability to book online.
  • Practice-initiated changes. If we must cancel or reschedule (illness, equipment outage, weather, force majeure), we will offer the next available slot or a full refund of the deposit at your option.

6. Mobile Ultrasound Service Area

Mobile ultrasound visits are offered within a defined radius of our Columbia, MD office. Service area is determined at the time of booking using the address you provide. We reserve the right to:

  • Decline mobile bookings outside our coverage area;
  • Apply a mileage / travel surcharge for addresses beyond our standard radius (disclosed at booking);
  • Reschedule a mobile visit if the visit address is determined to be unsafe, inaccessible, or unsuitable for clinical imaging.

For mobile visits, you are responsible for providing a private space with appropriate access, lighting, and a stable surface for the patient. A responsible adult must be present at the address.

7. Electronic Communications & E-SIGN Consent

By providing your email address or mobile number and using our Services, you consent to receive electronic communications from us (appointment confirmations, reminders, results notifications, receipts, and operational notices). You may withdraw consent for non-essential messages at any time; communications related to your care or the Services may continue.

You agree that electronic signatures, contracts, orders, and other records, and electronic delivery of notices, policies, and records of transactions initiated or completed through the Services, satisfy any legal requirement that such communications be in writing.

8. Acceptable Use

You agree not to:

  • Submit false, misleading, or fraudulent information when booking;
  • Use the Services to harass staff, other patients, or third parties;
  • Attempt to access, scrape, reverse-engineer, or interfere with the Site or its security controls;
  • Upload malicious code, run automated agents, or stress-test the booking system;
  • Use the Site or any content obtained from the Site for unauthorized commercial purposes.

9. Intellectual Property

All content on the Site — including text, images, logos, the “Ultra Stat Imaging” name and brand, service descriptions, and software — is owned by Ultra Stat Imaging or its licensors and is protected by copyright, trademark, and other intellectual property laws. Personal, non-commercial use is granted; all other rights are reserved.

10. Third-Party Services

The Services rely on third-party providers for hosting, payment processing, scheduling, communications, and analytics. Those providers are bound by their own terms and, where they handle protected health information, by Business Associate Agreements with us. We are not responsible for the independent acts, omissions, or service availability of third-party providers.

11. Disclaimer of Warranties

THE SITE AND SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE.” TO THE FULLEST EXTENT PERMITTED BY LAW, ULTRASTAT IMAGING DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE UNINTERRUPTED, ERROR-FREE, OR SECURE.

12. Limitation of Liability

TO THE FULLEST EXTENT PERMITTED BY LAW, ULTRASTAT IMAGING, ITS OWNERS, EMPLOYEES, AND CONTRACTORS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS OR REVENUE, ARISING OUT OF OR RELATED TO YOUR USE OF THE SITE OR THE SERVICES. OUR AGGREGATE LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATING TO THESE TERMS OR THE SERVICES WILL NOT EXCEED THE GREATER OF (A) THE TOTAL AMOUNT YOU PAID TO ULTRASTAT IMAGING IN THE TWELVE (12) MONTHS PRECEDING THE EVENT GIVING RISE TO THE CLAIM, OR (B) ONE HUNDRED DOLLARS ($100). NOTHING IN THESE TERMS LIMITS LIABILITY THAT CANNOT LAWFULLY BE LIMITED.

13. Indemnification

You agree to indemnify and hold harmless UltraStat Imaging and its owners, employees, and contractors from any claim, demand, loss, or expense (including reasonable attorneys’ fees) arising out of your breach of these Terms, your misuse of the Services, or your violation of any law or third-party right.

14. Governing Law & Venue

These Terms are governed by the laws of the State of Maryland, without regard to its conflict-of-laws rules. The exclusive venue for any dispute that is not subject to informal resolution will be the state or federal courts located in Howard County, Maryland, and you consent to the personal jurisdiction of those courts.

15. Changes to These Terms

We may update these Terms from time to time. Material changes will be posted on this page with a new effective date and, where appropriate, communicated by email. Your continued use of the Services after changes take effect constitutes acceptance of the updated Terms.
